Do you have, or are you being treated for, high blood pressure?

BMI > 35

Is your body mass index more than 35 kg/m²? (~5'6" + 217 lb, or 6'0" + 257 lb)

Age > 50

Are you older than 50 years?

Neck > 40 cm

Is the circumference of your neck more than 40 cm (about 16 inches)?

Male

Were you assigned male at birth? (OSA is statistically more common in men, though post-menopausal women approach equal prevalence.)

About STOP-BANG. Developed in 2008, validated in surgical, sleep-clinic, and general populations. Sensitivity for moderate-to-severe OSA at the ≥3 cutoff is around 90%. Specificity is lower — many people score 3+ without having OSA, so a positive screen leads to a sleep study, not directly to a diagnosis. The "B-A-N-G" half (BMI, age, neck, sex) carries more weight than the "S-T-O-P" half in some validated variants — if your B-A-N-G subscore is ≥3 alone, that's also considered high risk.
