PCOS Self-Check (Rotterdam)

Self-check, not a diagnosis. PCOS diagnosis under the Rotterdam criteria requires meeting 2 of 3 criteria AND excluding other causes — that's a clinician's call. This tool helps you organise what you already know about your symptoms before that conversation.
Menstrual cycles
Over the past 6–12 months, your periods have been:
Excess facial or body hair (hirsutism)
Coarse dark hair on the face (chin, upper lip, sideburns), chest, abdomen, or back. Male-pattern distribution. Distinct from typical body hair.
Persistent or adult-onset acne
Cystic / hormonal acne, especially along the jawline and chin. Persistent past adolescence.
Male-pattern scalp hair loss / thinning
Thinning at the crown or temples (rather than diffuse all-over thinning).
Unexplained weight gain or difficulty losing weight
Supportive — not a diagnostic criterion by itself, but common.
Blood test ever shown elevated testosterone or DHEAS
Biochemical hyperandrogenism. If you've had hormone labs done, you may know this; if not, mark No.
Pelvic ultrasound result
Have you had a pelvic ultrasound and what did it show? (PCO appearance = ≥20 follicles per ovary, or ovarian volume ≥10 mL.)
About PCOS. Polycystic ovary syndrome affects 6–13% of women of reproductive age and remains underdiagnosed. The Rotterdam criteria are the most-used diagnostic standard internationally (NIH and AE-PCOS criteria are alternatives). Long-term, PCOS increases risk for type 2 diabetes, cardiovascular disease, endometrial cancer, and infertility — but it's also highly manageable with the right treatment plan. If symptoms are interfering with your life or you're trying to conceive, the conversation with a PCP, OB/GYN, or endocrinologist is worth having.
